Instructions (just in case you prefer written):
Boot into recovery & make sure you have CWM based recovery v6.0.4.7 & above.
Follow the steps given below.
wipe data/ factory reset
wipe cache
advanced > wipe dalvik cache
mounts & storage > format /system
install zip > choose zip > flash Rom File
choose zip > flash Gapps package
Go back and hit "Reboot System Now".
